Wearing braces can ultimately give you a healthier and more attractive smile. In the short-term, though, your gums in particular may be in for a rough ride.
While we're all susceptible to gum disease, braces wearers are more likely to encounter it. This stems from two related factors: the difficulty braces pose to oral hygiene; and the potential irritation of soft tissues by the braces themselves.
The main cause for any form of gum disease is dental plaque, a thin bacterial film that accumulates on teeth. Removing plaque through brushing and flossing greatly reduces the risk of any dental disease. But braces wires and brackets make it difficult to brush and floss—as a result, some plaque deposits may escape cleaning, which makes a gum infection more likely.
To exacerbate this, braces hardware can irritate the gums and cause swelling and tissue overgrowth, also known as hyperplasia. The one-two punch of ineffective hygiene with hyperplasia are why braces wearers have a higher incidence of gum problems compared to the general population.
To guard against this, patients with braces need to be extra vigilant about keeping their teeth and gums clean of plaque. It may be helpful in this regard to use specialized tools like interproximal brushes with narrower bristle heads that are easier to maneuver around braces.
And rather than using traditional flossing thread, orthodontic patients may find it easier and more effective to use pre-loaded flossing picks or an entirely different method called oral irrigation. The latter involves a handheld wand that directs a stream of pulsating water between teeth to loosen and flush away plaque.
It's also important for patients to see their dentist as soon as possible for any gum swelling, bleeding or pain. The dentist can determine if it relates to gum disease, hyperplasia or a combination of both, and recommend treatment. In extreme cases, it may be necessary to remove the braces until the gums heal, so catching and treating any gum problem early is a priority.
Regardless of the risk for gum disease, orthodontic treatment is still well worth the investment in your health and appearance. Practicing effective oral hygiene and keeping a watchful eye on your gums will help further lower that risk.

There are few things sweeter to hear than for your dentist to tell you your periodontal (gum) disease is under control. Depending on how deep the infection may have advanced, your treatment journey may have been a long one.
Unfortunately, while the battle may be over, the threat still lingers—once you've experienced a gum infection, you're at higher risk for a recurrence. To minimize that risk, you may need to undergo dental cleanings on a more frequent basis than before.
The average patient typically sees their dentist for cleanings every six months. The aim of these visits is to remove dental plaque, a thin film of bacterial-laden particles that is the prime source for gum disease. These cleanings are meant to supplement a daily habit of brushing and flossing, which should remove the bulk of plaque that builds up throughout the day.
After gum disease treatment, though, you may need to have these cleanings more frequently, and of a more involved nature than the normal cleaning. For patients who've overcome advanced gum disease, that frequency could initially be every other week, every couple of months or every three months. This frequency may change depending on the status of your gum health.
Besides a thorough cleaning, a specialized periodontal maintenance visit may include other interventions. For example, your dentist may apply topical antibiotics or other anti-bacterial products to keep bacterial growth under control.
Protecting you from further gum infection isn't totally on your dentist's shoulders—you also have a role to play. You'll need to brush and floss your teeth thoroughly every day, along with using any other hygiene products prescribed or recommended by your dentist. Daily hygiene will help prevent the buildup of dental plaque and subsequent bacterial growth.
You'll also need to keep a watchful eye on your gums for any emerging signs of infection. If you begin to notice swelling, pain or bleeding, contact your dentist as soon as possible to initiate remedial treatment.
Gum disease treatment can bring your gums back to a reasonable state of good health. But that state could be reversed with a returning gum infection. Only vigilance practiced by both you and your dentist can stop that from happening.
Although getting an implant requires surgery, it's usually a minor affair. Chances are good that after just a few days recuperation you'll be back completely to your normal activities.
But like many other minor surgeries, an implant procedure does pose a slight risk of post-op infection. That's especially so with any dental procedure like implant surgery, since the mouth harbors numerous strains of bacteria that could escape into the bloodstream. For most people, though, a post-op infection doesn't pose a major problem since their immune system kicks in immediately to defeat it.
But some patients with less than robust immune systems or other health problems can have serious complications from an infection. Among other things, infected tissues around an implant may not heal properly, putting the implant at significant risk for failure.
If you have a condition that makes a post-op infection problematic, your dentist or physician may recommend you take an antibiotic before your procedure. Known as prophylactic (preventive) antibiotic treatment, it's intended to give a weakened immune system a head-start on any potential infection after a procedure.
Using antibiotics in this way has been a practice for several decades, and at one time were recommended for a wide list of conditions. That's changed in recent years, though, as evidence from numerous studies seems to show the risk to benefit ratio isn't significant enough to warrant its use in all but a handful of conditions.
Both the American Dental Association and the American Heart Association recommend prophylactic antibiotics for patients with prosthetic heart valves, past infective endocarditis, a heart transplant and some congenital heart conditions. Some orthopedists may also recommend it for patients with prosthetic joints.
Even if you don't fall into these particular categories, prophylactic antibiotics may still be beneficial if you have a compromised immune system or suffer from a disease like diabetes or lung disease. Whether or not a prophylactic antibiotic is a prudent step given your health status is a discussion you should have with both your physician and your dentist.
If they feel it's warranted, it can be done safely in recommended doses. If your health isn't as robust as it could be, the practice could give you a little added insurance toward a successful implant outcome.
